Completion Form

When you have completed your test, you may fax or mail your bubble-in answer sheet to the Testing Office.

Both the fax number and mailing address for the Testing Office are on your bubble-in answer sheet.

Upon passing the test with a 70% or higher (80% or higher for upgrade to Florida Supervisor), we will MAIL out your certificate of completion within 5 business days.
