HOW TO ORDER THE QUESTIONS IN #50-X BELOW
	Indicate the number of sets of questions you need, Write in the course #s of the answer sheet(s) you need questions for, Indicate how you want them sent, by E-mail or U.S. Mail.
A.	For One Answer Sheet - order one set of questions. Example 1: #01-D West Nile Virus has one answer sheet, so choose 1 set Ques from the drop down menu below and then put #01-D in the box below.
B.	For Multiple Answer Sheets, order one set of questions for EACH answer sheet. Example 2: #05-X, Gen Rev has 2 answer sheets, so choose 2 sets Ques from the drop down menu below and then put #05-X, Parts (whichever 2 parts you want) in the box below. Example 3: #12-B, HIV has 1 answer sheet and #14-G, Med Errors has 1 answer sheet, so choose 2 sets of Ques from the drop down menu below and then put #12-B & #14-G in the box. Example 4: #25-J Micro 1-2-3 has 3 answer sheets, so choose 3 sets Ques from the drop down menu and then put #25-J in the box.
C.	For Combo Courses - order one set of questions for EACH answer sheet. Example 5: #21-H, FL MEAL, has 3 answer sheets, so choose 3 sets Ques from the drop down menu and then put #21-H in the box. Example 6: #20-H, FL Renewal has 4 answer sheets so choose 4 sets Ques from the drop down menu and then put #20-H in the box. Example 7: #19-J, 5 Area Combo, has 5 answer sheets, so choose 5 sets Ques from the drop down menu and then put #19-J in the box
